Jenks man killed in accident at Paragon Industries in Sapulpa

OHP says the man fell off of a forklift and under the wheel

An industrial accident killed a young man at Paragon Industries in Sapulpa early Friday morning.

It happened a little after 3:00 a.m. according to the Oklahoma Highway Patrol, which investigated.

OHP says William Miller, 20, of Jenks had jumped onto the side of a forklift being driven by another man who was going to pick up some pipe.

Paragon is an oil pipeline manufacturing company.

He slipped and fell under the wheel of the forklift, which rolled over him.

Juan Rodriguez of OSHA tells FOX23 News' TiffanyAlaniz that they have three investigations on file regarding Paragon in recent years.

The most recent, in 2011, turned up three violations, but none of them serious.

The company was fined $2,700 according to OSHA records.

Paragon Industries has not responded to requests for comment.
