A dam on an eastern Iowa lake suffered a "catastrophic" failure Saturday, sending a massive amount of water into nearby communities and forcing residents to flee, officials said. The Lake Delhi dam, about 45 miles north of Cedar Rapids, failed as a result of "massive rain -- a very unusually high amount this season," according to Jim Flansburg, communications director for Gov. Chet Culver. Culver told CNN that nearly 10 inches of rain had recently fallen in a 12-hour period in the area and was "too much water for the dam to hold."
A six-year archive of classified military documents made public on Sunday offers an unvarnished, ground-level picture of the war in Afghanistan that is in many respects more grim than the official portrayal. The secret documents, released on the Internet by an organization called WikiLeaks, are a daily diary of an American-led force often starved for resources and attention as it struggled against an insurgency that grew larger, better coordinated and more deadly each year. The New York Times, the British newspaper The Guardian and the German magazine Der Spiegel were given access to the voluminous records several weeks ago on the condition that they not report on the material before Sunday. The documents -- some 92,000 reports spanning parts of two administrations from January 2004 through December 2009 -- illustrate in mosaic detail why, after the United States has spent almost $300 billion on the war in Afghanistan, the Taliban are stronger than at any time since 2001.
KOTV: A man was wounded when a gun discharged at an Oklahoma City gun show Sunday morning. Oklahoma City police say a vendor at the Metcalf Gun Show failed to empty the weapon and it accidentally discharged when he was handing it to a potential buyer, hitting the vendor's son at about 11:45 a.m. "The owner has admitted he simply failed to clear the gun. In fact it was his son who was shot in the knee," said Oklahoma City Police Lt. Jay Barnett. The 22-year-old victim was transported from the fairgrounds to an Oklahoma City hospital where he is expected to recover. Organizers said the man will not be allowed to operate a table at their shows again.
This fall, the flu shots that used to be free at the county health department will cost $25. The state found that most other states charge for the service and wants people who have insurance to start paying for the service. People on Medicare, Medicaid and with low incomes will get a fee waiver
A maintenance man at a prominent western New York office building was crushed to death after falling into a trash compactor, but no one realized it for weeks as his family and authorities searched for him, police said Sunday. Surveillance video shows John Adams tumbled into a compactor while working at the One Niagara building, and the mechanism "prevented him from being able to escape and made it impossible for co-workers or others to realize what had occurred," Niagara Falls, N.Y., police said in a release.
For the last week, the family and friends of 19-year-old Abby Guerra, who officials said died in a July 18 car crash, have been mourning. Yesterday, the promising young soccer star from Glendale, Ariz., was discovered to be in the hospital, alive. Authorities had confused Guerra with 21-year-old Marlena Cantu, another passenger in the vehicle who was thought to have survived the crash with severe injuries but in fact was killed. Cantu's family have sat at Guerra's bedside for the past week, believing she was Marlena, and now are left with the cruel revelation that their loved one actually died in the crash. The two women were traveling home from a day out at Disneyland in Southern California with three former high school friends when the left-rear tire of their SUV blew out, causing the vehicle to flip.
UK - Marks and Spencer has begun selling "plus-size" clothing for children as young as three in an attempt to dress a new generation with bulging waistlines. Health campaigners said the decision by the High Street giant demonstrated the scale of the obesity epidemic affecting Britain's "overindulged" children, with one in four children now classed as obese or overweight by the time they start primary school. The retailer said it began a trial of the "plus fit" range last week, in an attempt to meet consumer demand.
Arizona - Southwest Airlines recently added "mechanical difficulties" to the list of acts of God and other events for which the carrier will not be liable if travel is delayed. It appears on page 11 of 32 pages of fine print called a "contract of carriage," which many passengers don't read, but which spells out their recourse in mishaps such as flight interruptions or baggage loss. Mechanical difficulties - what type is not defined - now appears on a list of events such as wars, riots, storms, earthquakes and other acts of God that are "outside of Southwest's control.
Southwest Airlines made headlines earlier this year for kicking overweight actor-director Kevin Smith off a flight because he took up more than one seat. Now we're hearing the airline recently removed a 5-foot-4, 110-pound Sacramento-area woman from a plane so a hefty passenger could have an extra seat. The incident happened last week on an early-evening Southwest flight from Las Vegas to Sacramento. The local woman was flying standby, paid full fare for the last available seat, got on board, stowed her bags and sat down - only to be told she would have to deplane immediately. The reason? A late-arriving passenger required two seats because of her girth. The Sacramento woman, a frequent-flying sales rep, was stunned.
TMZ: The investigation into Brittany Murphy and Simon Monjack's deaths is now being handled by the Department of Public Health, TMZ has learned ... and it's all because of mold found in their home. Law enforcement sources tell TMZ the issue of mold in the house came up early during the investigation into Brittany's death -- but it was deemed "not to be a contributing factor." Then when Simon passed away a few months later, we're told the issue came up again -- but Brittany's mom, Sharon, wouldn't let the L.A. County Coroner's Office in to investigate the mold. Now, we're told, Sharon has agreed to let Public Health investigators into the home. Mold could be relevant to Simon and Brittany's respiratory health. Pneumonia was a cause of death for both of them.

The popular and Grammy award-winning band Kings of Leon were forced to end a concert in St. Louis last night after playing just three songs because a pigeon pooped into bassist Jared Followill's mouth. A spokesman for the Verizon Wireless Amphitheater told the crowd the show would not be able continue due to fears for the band's safety. Drummer Nathan Followill also apologized to fans on Twitter: "So sorry St. Louis.
